Reablement Support Worker to join our responsive, multi-disciplinary Reablement Team on a
temporary basis.You'll provide hands-on, person-centred care and therapeutic support to individuals following illness, injury, or hospital discharge - helping them rebuild their independence and continue living safely in their own homes.
Your Key Responsibilities: - Deliver personal, practical, and rehabilitative care in line with individualised care plans.
- Carry out therapeutic activities and skills practice to improve mobility, daily living, transfers, and confidence.
- Support clients to manage their home safely, maintain routines, and re-engage with social contacts and activities.
- Accurately record and report changes in individual health, wellbeing, or circumstances to senior staff.
- Communicate effectively - verbally and in writing - to ensure safe and coordinated care delivery.
- Work as part of a 7-day service, ensuring clients receive timely support when they need it most.
What We're Looking For: - Valid UK driving licence and daily use of your own vehicle (essential for travelling across the Borough).
- Experience working in a care, community, or reablement setting (desirable).
- Ability to work independently and as part of a team, using initiative to report and act on concerns.
- Good communication skills - both verbal and written - with confidence using IT and electronic monitoring systems.
- Experience working with professionals, families, and other agencies is a plus.
