This role involves providing statutory social work services to adults with mental health needs across Croydon. You will conduct Care Act 2014 assessments, manage risk effectively, and deliver person-centred care planning to promote independence, safeguard vulnerable adults, and ensure effective collaboration with health and community partners.
Key ResponsibilitiesUndertake comprehensive assessments under the Care Act 2014.
Develop and review personalised care and support plans to promote independence and recovery.
Complete Mental Capacity Act assessments and apply its principles in practice.
Identify safeguarding concerns and participate in relevant enquiries and risk management processes.
Manage a varied caseload efficiently, maintaining accurate and timely records.
Work closely with NHS and Community Mental Health Teams to integrate support.
Apply a recovery-focused, strengths-based approach in all interventions.
Ensure compliance with the Care Act 2014, Mental Health Act 1983, and council procedures.
Degree or Diploma in Social Work (or equivalent) with Social Work England registration.
Experience in adult social care or mental health services, with knowledge of relevant legislation.
Strong assessment, analytical, and case management skills.
Commitment to anti-discriminatory practice, equality, and empowerment.
Emotional resilience and the ability to manage challenging cases.
