Social Worker to join our
Hospital Integrated & Community Discharge Team within Adult Social Care. This is an exciting opportunity to work in a fast-paced, supportive environment that values professional growth and collaborative working.
About the Role As a key member of our integrated discharge team, you'll play a vital role in ensuring timely and safe hospital discharges. You'll work across acute and community settings - including Medway Hospital, out-of-area hospitals, step-down beds, and service users' own homes - supporting people through their discharge and recovery journey.This role also includes supporting the Emergency Department, working closely with health professionals to enable efficient transitions of care. You'll adopt a
strengths-based, person-centred approach, ensuring all interventions promote independence and achieve positive outcomes.
Key Responsibilities - Carry out assessments in line with the Care Act 2014, Mental Capacity Act, and Mental Health Act.
- Facilitate safe, timely discharges from hospital settings through the Discharge to Assess (D2A) model.
- Collaborate with health and social care partners to coordinate effective support plans.
- Promote safeguarding and ensure the welfare of all service users.
- Work flexibly across hospital and community settings as service demands evolve.
About You You'll be a
qualified Social Worker with current registration (Social Work England), ideally with experience in hospital discharge, community, or integrated care settings. You'll have strong assessment and communication skills, with a commitment to collaborative working and person-centred practice.
