Our Hospital Discharge Transfers of Care Unit is looking for a passionate and dedicated Weekend Deputy Team Manager to join our team and help make a difference today!

About the Role:

As a Weekend Deputy Team Manager, you'll play a vital role in leading our hospital discharge service during the weekend period, working Tuesday to Saturday (36 hours).

You'll:

  • Provide day-to-day operational leadership across the team, including oversight of triage, case allocation, and assessment approvals.
  • Offer expert advice on complex discharges, safeguarding concerns, and mental capacity assessments.
  • Act as the lead manager for weekend discharges from Kings and St Thomas Hospitals.
  • Manage step-down placements into extra care schemes, ensuring efficient referral and placement processes.
  • Supervise and support social workers and assistant practitioners, promoting reflective practice and continuous professional development.

This is a key leadership post where your decisions and support will directly impact timely and safe discharges for Southwark residents.

About You:

We're looking for a confident, experienced social care professional with the following:

  • Qualified Social Worker status and registration with Social Work England.
  • Strong working knowledge of the Care Act, Mental Capacity Act, and Continuing Healthcare.
  • Experience leading multi-disciplinary meetings, safeguarding cases, and professional supervision.
  • A proactive and calm leadership style with the ability to make decisions under pressure.
  • Desirable (but not essential): Experience as a Practice Educator or working with ASYE staff.

Our Hospital Discharge Transfers of Care Unit plays a critical role in ensuring seamless transitions from hospital to home or care settings. We manage complex discharges and safeguarding matters with a resident-first approach. The weekend team is a vital extension of our weekday service-committed, resilient, and focused on delivering high-quality care when it matters most.

Additional Information:

  • This is a 36-hour role, covering Tuesday to Saturday.
  • The post attracts time-and-a-half pay for weekend work completed.
  • This is a qualified social worker role, and leadership experience in adult social care is essential.
  • Requires a satisfactory Disclosure and Barring Service (DBS) DBS check at Enhanced level.
