Location: East Herts, Broxbourne & Welwyn Hatfield ? Directorate: Adult Care Services Service Area: East and North Older People's Service Pay Rate: Up to £34.00 per hour (depending on experience) Working Pattern: Hybrid working Hours: Mon-Thurs 9:00-17:30 Fri 9:00-16:30 DBS Required: Yes

Join our dedicated Older People's Social Work Team as a Social Worker, supporting adults across East Herts, Broxbourne, and Welwyn Hatfield. This 6-month contract is funded through winter pressures, offering a valuable opportunity to make a meaningful impact during a critical time.

• Based in the locality, supporting people in their own communities

• ? Help reduce hospital admissions through timely intervention

• Complete Care Act assessments and reviews

• Arrange and coordinate appropriate care and support packages

• Act as Safeguarding Enquiry Officer

• Undertake Mental Capacity Assessments and Best Interest decisions

• Work collaboratively with professionals across health and social care

• ? Qualified Social Worker

• Registered with Social Work England

• Full UK driving licence

• Strong organisational, communication, and IT skills

• Experience working with older adults, including those with cognitive impairments
