The Role

As an Adults Localities Social Worker, you will be part of an integrated and holistic team, working closely with colleagues from the NHS and other partners. Your role will be varied and fast-paced, providing you with the opportunity to utilize your full range of social work skills.

Key Responsibilities:

• Comprehensive Assessments: Undertaking Care Act assessments and reviews to identify and address individuals' care and support needs.
• Safeguarding: Leading on safeguarding enquiries to protect vulnerable adults from abuse or neglect.
• Capacity and Best Interests: Carrying out Mental Capacity Act assessments and acting in a person's best interests where they lack the capacity to make their own decisions.
• Continuing Healthcare: Completing Continuing Healthcare (CHC) checklists and attending Decision Support Tool (DST) meetings to ensure people receive the right funding and care.
• Duty and Case Management: Managing a varied caseload and participating in the general duty rota to respond to urgent and complex referrals.
• Collaboration: Working collaboratively with multi-disciplinary teams, families, and carers to develop and implement person-centred care plans.

About You

You will be a qualified and registered Social Worker with Social Work England and have a proven track record of working in a statutory adult social care setting. You will be confident in your ability to manage a busy caseload, assess complex situations, and make sound, defensible decisions.
