CHC Nurse Assessor Band 6Location: EssexRate: £300 per dayWorking Pattern: Monday to Friday, 9:00am - 5:00pmTeam: Continuing Healthcare (CHC)

• Conduct thorough face-to-face assessments, reviews, and monitoring of care for clients funded via NHS Continuing Healthcare and Funded Nursing Care

• Act as Case Manager to facilitate, plan, and evaluate client care in collaboration with families and professionals

• Maintain strong relationships with health, social care, and independent sector colleagues

• Provide professional advice, training, and mentorship to colleagues where required

• Participate in service development, clinical audits, and research to improve practice and patient outcomes

• Ensure all assessments are completed in line with national frameworks, safeguarding, and statutory guidance

? Registered Nurse with current NMC registration (RGN, RMN or RNLD)? Previous experience conducting Continuing Healthcare assessments for adults with complex needs? Strong knowledge of MDT working, safeguarding, and clinical governance? Excellent communication, negotiation, and conflict management skills? Ability to manage complex caseloads and work autonomously? Experience in providing advice and support across health, social care, and independent sectors

Desirable:? Educated to degree level (or equivalent experience in a specialist area)? Master's in healthcare or related area? Experience in leadership roles and cross-boundary collaboration? Knowledge of commissioning arrangements, DOLS, and SEND agenda

Monday to Friday, 9:00am - 5:00pmContract: 3-6 months ongoing Immediate to 4-week start
