Salary per annum: £39,152 - £45,091 per annum
Woring Arrangements: Full time, Hybrid Working Available We are recruiting for a permanent Adult Social Worker to join the Aging Well Adults' Community Team in Southend-on-Sea. This is an excellent opportunity to work within a supportive team, making a real difference to older adults and promoting independence within the community.
About the role:- Undertake assessments and reviews for older adults and older adults experiencing mental ill health.
- Apply a strengths-based approach to support individuals in maintaining independence and living safely within the community.
- Develop and review person-centred care and support plans that reflect individual needs, goals, and desired outcomes.
- Work collaboratively with service users, carers, families, health professionals, and partner agencies to deliver effective support.
- Identify and build upon individual strengths, community resources, and support networks to promote wellbeing.
- Support and empower carers to sustain their caring role and access appropriate support services.
- Carry out safeguarding duties, risk assessments, and interventions in line with statutory responsibilities and legislation.
- Promote independence by enabling individuals to make informed decisions about their care and support.
- Manage a varied caseload, maintaining accurate and timely case records.
- Participate in multidisciplinary meetings and contribute to integrated health and social care working.
- Ensure compliance with relevant legislation, policies, and professional standards.
- Contribute to service development and continuous improvement initiatives within the Ageing Well Service.
About you:- Qualified Social Worker with Social Work England registration
- Experience working within Adult Social Care
- Strong assessment and care management skills
- Full UK driving licence and access to a vehicle if you do not live locally to Southend
