Care Plan Writer

BDS are recruiting for an experienced care plan and risk assessment writer to be based within an Extra Care service based in Lewisham.

This is an exciting opportunity to work with one of the leading Housing Associations in the UK writing person centered care plans and risk assessments to support the residents and their families of the service. We have 2 roles- 1 based in SE16 and 1 in SE12.

Main duties will include but are not limited too;

• To meet with new and existing residents of the service and family members to assess the daily needs and requirements of each person.
• Identifying and risks or hazards
• Writing the care plan to match all needs and requirements
• Writing risk assessments as you find them,
• Updating risk assessments and care plans as required, meeting the ever-changing needs of the residents.
• You will also be required to complete mental capacity assessments, individual assessments and best interest decisions.

The ideal candidate will;

• Have experience in this position previously or suitable experience in a previous role.
• Have an enhanced DBS check
• Be flexible in your approach to the role, meeting the needs and requirements of the service.

Hours; Monday to Friday 9 - 5pm (can be flexible!)

This is a temp ongoing role to start ASAP!

Pay rate from 21.50 per hour on a PAYE basis.
