• Position: Supported Pathways Officer
• Working Hours: Shift patterns Monday to Friday

What You'll Do:

• Support individuals in their journey to independence, providing tailored advice and assistance.
• Facilitate access to services, resources, and opportunities that enhance personal and professional growth.
• Monitor and assess progress, offering ongoing guidance and support.
• Being based at the hostels in Brent on a shift pattern basis
• Work collaboratively with a network of professionals to ensure the best outcomes for those in need.

• Strong interpersonal and communication skills.
• A compassionate, patient, and supportive nature.
• Experience in a social care, support, or community-focused role (desirable but not essential).
• Ability to work effectively within a team and independently.
