Day/Night Support Co--Ordinator

This role is perfect for compassionate, motivated individuals who want to support people to live independently, with dignity and respect.

• Support adults with disabilities to take control of their lives and meet their planned goals and outcomes.
• Provide high-quality support, including personal care, emotional support, and day-to-day living assistance.
• Promote independence, wellbeing, and meaningful engagement with the community.

• Participate in care planning, monitoring, and reviews.
• Safeguard vulnerable adults in line with council policies.
• Support individuals to maintain relationships and wellbeing.
• Organise and take part in activities within the service and community.
• Provide personal care, moving/handling support, and administer medication.
• Record information clearly and accurately, reporting any concerns.
• Work flexibly across a 7-day rota including evenings, weekends, bank holidays, and occasional sleep-ins.

• NVQ Level 2 in Health & Social Care (or equivalent).
• Knowledge of safeguarding adults.
• Manual handling skills.
• Safe handling and administration of medication.
• Awareness of food hygiene and infection control.
• Strong communication skills and ability to record information clearly.
• Understanding of personalisation and person-centred approaches.

• NVQ Level 3 or above in Health & Social Care.
• Experience supporting people with learning disabilities, autism, epilepsy, or other long-term conditions.
• Experience with lone working and challenging behaviour.
• Knowledge of "Valuing People Now" and current social care practice.
• Ability to plan activities and support daily living.
