Adult's Social Worker, Learning Disability / Transitions Team, South East Pay rate £37 per hour Contract role, LD team Pertemps are recruiting for an experienced Adult's Social Worker in the South East This is to work in a Learning Disability / Transitions Team. The Transitions Team work with young people, their families, education, health and further networks to support the transition into Adult Social Care. This involves Care Act Assessments, just prior to 18th birthdays. Ideal locations for this role include Bedfordshire, Hertfordshire, Buckinghamshire, Milton Keynes, Oxfordshire, and there is plenty of hybrid working available.
