Reablement Worker
Hammersmith & Fulham (community work in the resident homes)
3 months initially
Rota basis from Monday to Sunday, 36 hrs/week must be flexible
- Provide a high standard of reablement care and non-invasive health treatment for clients referred into the Community Independence Service
- Provide a dignified and respectful service that encourages and motivates disabled, frail and older people, discharged from hospital, to live active lives of their choosing
- Focus on the needs of the individual, promoting independence and choice and maximises quality of life
- Follow reablement goal plans and offer a programme of individualised services to clients
- Support with personal care tasks such as bathing, dressing
- Provide practical support such as house work, preparing meals
- Support with non-invasive health tasks such as medication support, changing catheter bags
- Support with mobility and walking exercise
- Monitor the progress of clients, keeping stakeholders informed of any changes
- Work in partnership with other teams and agencies to maximise reablement potential and support the transition into and out of the Community Independence Service
- Attend meetings where required
Requirements
- Minimum of an NVQ Level 2 in Adult Social Care (desired)
- Experience gained of caring for vulnerable people and an understanding of their needs
- Ability to understand the process of assessment, planning, implementation and evaluation of care and follow a reablement goal plan
- Manual Handling for when needed
- Health and Medical Fitness
- Enhanced Adult DBS Certificate registered to the update service
