Our Intermediate Care Service (ICS) is looking for a passionate and dedicated Assistant Practitioner (Social Work) to join our Reablement Team and help make a real difference every day.

You will work alongside one other Assistant Practitioner, Customer Support Officers, and a Deputy Team Manager.
