About the team

IDS West

Watford General Hospital

37 hours per week

Watford General is a 7 day working Hospital. This is a 37 hour a week position, and 1 in every 8 weekends.

Focusing on supporting people to leave hospital as quickly and safely as possible with the majority of the work being through Discharge to Assess.

The usual hours are 9am – 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role.

IDS West

Herts Valley Community Hospital Team - Potters Bar

37 hours per week

The service supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.

We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.

About the role

You will need to have knowledge and experience of relevant legislation. You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to within our Connected Lives Framework.

About you

Essential:

  • Paid or voluntary work experience in social or health care within the community
  • Driving licence and ability to travel.
  • An accredited RQF (registered qualification framework) level 4 social care qualification or equivalent. An equivalent would be a course that has aligned learning outcomes to the RQF level 4 social care qualification and involves an extended period of study with a supervised practice placement in a health, education or social care setting.

If you do not hold such qualification, there is an expectation that you will work towards gaining this within the first year of being in post. The qualification is available via an apprenticeship route. Maths and English at level 2 (GCSE/O’level A-C/CSE grade 1) is a requirement.

Desirable:

  • Knowledge and understanding of safeguarding inline with the Care Act 2014.
  • Knowledge of other social care practices.
