We are looking for a Social Worker to join a a secure mental health inpatient unit for older adults (male and female).
This role requires a Social Work Qualification with a minimum of 3 years post qualified experience.
About the team
Attend regular ward rounds as part of the multidisciplinary team, contributing to patient reviews and discharge planningCoordinate safe, timely discharges for patients once they are mentally well - sourcing the right onward setting, whether that's a care home, residential placement, or a return homeExtensive joint working with Adult Social Care, including requesting and coordinating Care Act assessments to support discharge planningWork with patients subject to Ministry of Justice restrictions under Sections 37/41 of the Mental Health Act, navigating the associated legal frameworkManage Section 17 leave applications and discharge processes as part of the patient pathwayA role suited to a social worker confident in mental health legislation, multi-agency coordination, and person-centred discharge planning
About you
A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years experience is essential in order to be considered for this role. Experience in a mental health setting and working with older adults.
