Onsite Clinical Pharmacist

An exciting opportunity has arisen for a Clinical Pharmacist to join a well-established GP practice in Hemel Hempstead. The practice is seeking a highly motivated and skilled individual who is passionate about patient care and improving the quality of healthcare delivery. As a Clinical Pharmacist, the successful candidate will work closely with the practice's clinical team to provide expert medication management, improve patient outcomes, and support the delivery of high-quality healthcare services.

The successful candidate will be supported by an excellent team of administrative and clinical staff which includes, GPs, Clinical Pharmacists, Nursing Team, Physiotherapist, Mental Health Nurse, Health and Wellbeing, and Midwife.

Routine responsibilities include:

• Review patient meds list.
• Provide expert clinical advise to the team on medication-related matters.
• Support the safe and effective prescribing of medicines.
• Deliver targeted interventions.
• Lead or participate in the development of clinical protocols relating to meds management.
• Contribute to the training and development of the practice team.
