We are currently seeking a highly skilled and compassionate Care Coordinator to join our domiciliary care service based in Tunbridge Wells. As the Care Coordinator, you will play a crucial role in ensuring the smooth and efficient delivery of care services to our clients.
Key Responsibilities:
Coordinate and manage the delivery of care services to clients in the communityCreate and maintain care schedules for clients, ensuring all shifts are coveredLiaise with clients, their families, and healthcare professionals to assess and develop care plansConduct regular reviews of care plans and make necessary adjustmentsMonitor and maintain accurate records of client care and staff schedulesProvide support and guidance to care staff, ensuring they are trained and equipped to deliver high-quality careConduct regular quality assurance checks to ensure compliance with regulations and standardsParticipate in recruitment and training of new care staffHandle any client or staff concerns or complaints in a timely and professional mannerCollaborate with other healthcare professionals to ensure the best possible care for clientsRequirements:
