Join our team as a Band 6 Social Worker and play a key role in supporting the older adults with mental health needs across three integrated teams in Bromley. In this role, you will:

• perform comprehensive patient assessments

• Caseload holding in a specialist older adult mental health team

• create and implement personalised care plans

• deliver evidence-based interventions to optimise outcomes

• collaborate with multidisciplinary teams to provide holistic care

• maintain accurate patient records

To be successful in this role, you will need:

• a current and valid SWE registration

• experience working with adults or older adults in mental health services

• strong communication and care planning skills

• the ability to work independently and within a multidisciplinary team

• confidence working in a fast-paced and integrated setting

This flexible working role requires availability from Monday to Friday, with part-time options considered. The contract is for 3 months with a strong possibility of extension based on performance and service needs.
