Reablement Support Worker

We are looking for a London Borough Of Merton will be based: Floor 3rd floor, Civic Centre, SM4 5DX

Reablement Officers will have to work 2 shift and rolling rota

Morning shift: 07:30am - 3:30PM

Evening Shift: 1:00pm - 09:00pm.

Need to work at least 2 weekend in a month

DRIVING LICENSE IS ESSENTIAL

Key Responsibilities:

• To support the Team leader in ensuring that people are enabled to achieve or regain maximum skills for independent living in the community.

• To provide a person centered approach and promote independence and empowerment to support people to attain their full potential.

• To develop and ensure the service focus's on a restorative approach to maximizing an individual's independence and enable them to link back into their community.

• To provide direct management support to Support Workers and work on a rota basis to ensure a seven day service can be provided.

• Undertake the Reablement assessment of current function and ability. Complete care and support plan for customers identified with Care Act eligible needs.

• Provide direct 'hands on' support to individuals when setting up the intervention plan to ensure its relevance and appropriateness. This may include personal care, cooking, cleaning, or any other daily living skill deemed necessary.

• Develop individual outcome focused Reablement goal plans as part of an intervention plan for individuals that will enable them to regain independence and reable them back into or remain living within their own home or community.

Essential Personal Specification:

• Level 2 or 3 NVQ or the willingness to undertake an NVQ/Care Certificate or other form of Comparable training deemed relevant to the post.
• Ability to apply critical analysis to a situation and identify robust solutions to identified problems.
• An ability to compile detailed written reports.
• An ability to follow procedural guidelines and complete appropriate documentation in the required formats.
• Ability to develop and implement goal plans and intervention plans that focus on outcomes which reflect individual's needs and preferences and maximize and maintain their personal independence.
• The ability to undertake specialist Reablement assessments.
