Job Overview:Join an ambitious, caring, and inclusive community team to support reablement and rehabilitation across Southwark. This role focuses on delivering home-based therapy for patients with complex needs.

Key Responsibilities:

• Manage a caseload of highly complex patients
• Complete OT assessments in client homes
• Set client-centred goals and implement tailored rehab plans
• Collaborate within a multi-disciplinary team to ensure cohesive care

Requirements:

• HCPC-registered Occupational Therapist
• Minimum 6 months of NHS experience, preferably in community rehab
• Passion for reablement and empowering independence
