Job Description:

Join our Community Social Work Team (CSWT) in Kingshill, Kent, and help provide essential support to vulnerable individuals and families in the community.

Key Responsibilities:

• Conduct comprehensive assessments to determine care needs.
• Develop and implement care plans, ensuring they meet the needs of service users.
• Provide ongoing support to individuals, helping them achieve independence.
• Ensure that safeguarding and risk management procedures are followed.
• Work alongside healthcare professionals and agencies to ensure seamless service delivery.

Qualifications & Requirements:

• Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work)
• Essential: Registration with Social Work England (SWE).
• Previous experience in a similar role, with a strong understanding of community-based social work.
• Excellent interpersonal and communication skills.
• Strong time management and organizational skills.
