Social Worker - Adults (Hospital Discharge)

Hospital-based (with flexibility) Monday to Friday 09:00 - 17:00 Up to £48.39/hr (Umbrella)

We're looking for a dynamic and experienced Social Worker to join our Hospital Discharge Team, supporting adults with timely, safe discharges and person-centred care planning

Carry out assessments & reviews under the Care Act ? Be present on-site at the hospital when required Attend Decision Support Tool (DST) meetings Support with hospital discharges and multi-agency working Work closely with health professionals & families

? Qualified Social Worker (SWE registered) Minimum 2 years of hospital social work experience Sound understanding of the Care Act 2014 Knowledge of NHS Continuing Healthcare & DST Enhanced DBS (adults)
