Pay Rate: Up to 38 p/h

Job Description:

We are looking for a skilled Social Worker to join our Community Social Work Team (CSWT) in Gravesend, Kent. This role will require the ability to assess and manage a caseload of individuals with a variety of needs.

Key Responsibilities:

• Assess the needs of vulnerable individuals and families in the community.
• Develop care plans and strategies for service users to achieve positive outcomes.
• Collaborate with local agencies and professionals to deliver holistic care.
• Ensure compliance with safeguarding and risk management policies.
• Maintain accurate case records and documentation.

Qualifications & Requirements:

• Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work)
• Essential: Registration with Social Work England (SWE).
• Experience working in a community-based social work setting.
• Ability to work under pressure and manage a busy caseload.
• Strong communication and interpersonal skills.
