Adults Social Worker - London Borough of Lambeth

Up to £32 per hour

The London Borough of Lambeth is looking for an experienced and dedicated Adults Social Worker to join our team. The successful candidate will be responsible for a range of critical duties, including attending the office to cover duty and support the team.

This role is vital for ensuring the well-being and safety of vulnerable adults in the community. You will be instrumental in undertaking essential Care Act and Mental Capacity assessments and determining s42 thresholds for adult safeguarding concerns.

Key Responsibilities

As an Adults Social Worker, you will:

• Manage a diverse caseload that includes vulnerable individuals and those with complex needs. You will be expected to take prompt action to protect individuals and others in emergency situations.
• Conduct thorough assessments of social care needs within specified timeframes, collaborating with health and other relevant agencies. You will always prioritize the wishes of individuals and their carers.
• Develop and implement effective care plans to meet assessed needs. This involves working closely with individuals, their relatives, carers, advocates, and other agencies.
• Secure the delivery of services within the allocated financial resources.
• Coordinate, monitor, and review care plans and service standards, and identify any gaps in service provision.
• Maintain up-to-date case records in line with national and local policies, and write professional reports for management.
• Ensure that all assessments and care plans take into account the views of users and their carers, empowering them to participate fully in the process.

About You

You will have a lead role in assessments, paying particular attention to an individual's strengths and weaknesses. You will also be adept at negotiating with users and carers to develop and implement comprehensive care plans.
